Plane too upright??
I'm personally working on this issue & it may be something to consider. If you get to upright or outside of the swing plane, it makes it almost impossible to get back to the true plane and impossoble to hit the inside/aft part of the ball.

Another thing to consider is the right elbow. I swing, but have some right hand hitting tendencies. Just before impact I want to hit it and my right elbow extends completely "a pre-mature both arms straight condition" and it will dig deep behind the ball & will close the club face.

What is your ball flight with short, medium, & long clubs? Good luck!!!
