Need some help with keeping my right shoulder on plane

Need some help with keeping my right shoulder on plane
Well i took some video of myself the other day and was pleasantly surprised. Lots of times your swing looks better in your head than it does on video, well it was pretty close to what i envisioned in my head which was nice

However i found out why my divots tend to point a little left. Conventional wisdom would tell you i have a bent plane line and am coming over the top. Nope.

What's happening is that during impact, instead of the right should continuing to go DOWNPLANE it starts to head left much too soon. This all makes sense because my divots start to point right (you can actually see it in the grass) but then they curve left pretty quick.

I also have a feeling this is why i fade the ball so easy as well.

What are some drills that i can do to get rid of this?
